Part 1 — Guest Agreement
This Guest Consent, Media Release & Liability Waiver (“Agreement”) is entered into between Tribute to America’s Fallen Foundation LLC, producer of the Meet a Hero podcast (“Producer,” “we,” “us,” or “our”), and the individual identified above (“Guest,” “I,” or “me”) in connection with Guest’s appearance on Meet a Hero.
1. Purpose of Interview. Meet a Hero is a podcast produced by Tribute to America’s Fallen Foundation LLC to honor military service members, veterans, and Gold Star families by sharing their stories. By signing below, I agree to be interviewed and/or recorded for use in this podcast and related promotional materials.
2. Voluntary Participation & Right to Pause. My participation is entirely voluntary. I understand that topics discussed may include deployment, combat, loss of a loved one, grief, or other sensitive subject matter. I may decline to answer any question, request a pause, or end the interview at any time without penalty.
3. Consent to Record. I consent to being audio- and/or video-recorded, photographed, and/or livestreamed for purposes of this podcast episode and related content.
4. Grant of Rights / Media License. I grant Producer, Meet a Hero, and its founders, hosts, employees, volunteers, and assigns a perpetual, worldwide, royalty-free, non-exclusive license to record, edit, reproduce, publish, broadcast, distribute, and display my name, voice, image, likeness, biographical information, and statements (“My Materials”) in any format or medium now known or later developed — including audio/video episodes, social media, websites, printed materials, and promotional content — in furtherance of the Foundation’s mission.
5. No Guarantee of Publication; Editorial Control. Producer has sole discretion over whether, when, and how any recorded material is edited, published, or used, and is under no obligation to publish or continue distributing my interview once published. Producer may edit My Materials for length, clarity, or content.
6. No Compensation. I understand I am participating voluntarily and will not receive monetary compensation for my appearance unless otherwise agreed to in a separate signed writing.
7. Ownership. The recording and any resulting edited content are the property of Tribute to America’s Fallen Foundation LLC. I will not receive royalties or ownership rights in the finished product.
8. Release and Waiver of Liability. In consideration for being invited to participate in Meet a Hero, I, on behalf of myself and my heirs, executors, and assigns, release and, Meet a Hero, and its founders, hosts, officers, directors, employees, volunteers, and agents (the “Released Parties”) from any and all claims, demands, damages, or causes of action of any kind, known or unknown, arising out of or related to my participation in the interview, the recording process, or the use, broadcast, or publication of My Materials — including claims for defamation, invasion of privacy, right of publicity, or emotional distress — except for claims arising from the Released Parties’ gross negligence or willful misconduct.
9. Accuracy of Statements; Opinions. I am solely responsible for the truthfulness and accuracy of any statements, stories, or opinions I share. Views and opinions I express are my own and do not necessarily reflect the views of Tribute to America’s Fallen Foundation LLC or Meet a Hero.
10. Sensitive Subject Matter Acknowledgment. Because Meet a Hero often addresses military service, combat experience, and the loss of service members, I acknowledge that discussing these topics may bring up difficult emotions. I confirm I am participating of my own free will and that Producer has not pressured me to disclose information I am uncomfortable sharing.
11. Privacy Requests. If there is specific information (e.g., names of surviving family members, exact dates, addresses, or unit/deployment details) I do not want shared publicly, I will identify it in the space below or notify Producer in writing before publication. Producer will make reasonable efforts to honor such requests but cannot guarantee omission once material has already been distributed.

12. Minors. If Guest is under 18 years of age, this Agreement must be signed by a parent or legal guardian on the minor’s behalf.
13. Right to Withdraw Consent. I may request that Producer remove previously unpublished raw material from consideration at any time before publication by notifying Producer in writing. Once an episode has been published and distributed, Producer cannot guarantee removal from all platforms, mirrors, or third-party shares.
14. Indemnification. I agree to indemnify and hold harmless the Released Parties from any claims arising out of statements I make during the interview that are false, defamatory, or that infringe upon the rights of a third party.
15. Governing Law. This Agreement is governed by the laws of the State of Georgia, without regard to conflict-of-law principles.
16. Entire Agreement. This document represents the entire agreement between Guest and Producer regarding participation in the Meet a Hero podcast and supersedes any prior oral or written understanding on this subject.

Part 2 — GoldStars Tribute Wall: Memorial Display Photo & Video Consent
This section applies when photographs, video, written tributes, or personal memorial items are submitted or made available for display on the GoldStars Tribute Wall (physical memorial displays shown at ceremonies, parades, schools, and community events, and in related photos, video, printed programs, the Foundation website, and social media).
1. Submission of Materials. I submit or authorize the use of photographs, video, written tributes, biographical details, and/or personal memorial items (“Memorial Materials”) depicting or relating to the service member named above, for inclusion on the GoldStars Tribute Wall.
2. Authority to Submit. I represent that I am a family member, legal representative, or otherwise have the right and authority to submit these Memorial Materials, and that I have obtained any necessary permission from other family members before submitting.
3. License to Display & Reproduce. I grant Tribute to America’s Fallen Foundation LLC a non-exclusive, royalty-free, worldwide license to reproduce, print, mount, laminate, engrave, and publicly display the Memorial Materials on physical Tribute Wall displays — including at memorial events, parades, ceremonies, schools, and other community locations — and in related photographs, video, printed programs, the Foundation’s website, and social media, for the purpose of honoring and remembering the service member.
4. Careful Handling; No Guarantee Against Loss or Damage. Tribute to America’s Fallen Foundation LLC will use reasonable care in handling, transporting, storing, and displaying Memorial Materials. I understand that physical items (original photos, mementos, flags, medals, uniforms) carry inherent risk of damage, loss, or wear from repeated transport, display, and handling, and I release Tribute to America’s Fallen Foundation LLC from liability for such damage or loss except in cases of gross negligence or willful misconduct. Where possible, I am encouraged to provide digital copies or reproductions rather than irreplaceable originals.
5. Ownership & Return of Original Items. Any original physical items I provide remain my property unless I state otherwise in writing. I may request return of original items at any time, and Tribute to America’s Fallen Foundation LLC will make reasonable efforts to return them within 30 days, recognizing that items incorporated into a physical display (e.g., laminated or mounted) may not be separable without damage.
6. Duration of Display. Memorial Materials may remain part of the Tribute Wall indefinitely unless I request removal in writing. Tribute to America’s Fallen Foundation LLC will make reasonable efforts to honor removal requests within a reasonable time, understanding some materials already incorporated into printed or laminated displays may be impractical to individually modify.
7. Accuracy of Information. I am responsible for the accuracy of names, dates, ranks, units, and other biographical details I provide about the honored service member, and I will notify the Foundation promptly of any needed corrections.
8. No Compensation. I understand that submitting and displaying Memorial Materials is a voluntary act of tribute, and neither I nor the honored service member’s estate will receive compensation.
9. Emotional Sensitivity Acknowledgment. I understand that creating, submitting, and viewing memorial displays honoring a fallen service member may bring up grief and difficult emotions, and I am submitting these Memorial Materials of my own free will as a tribute to my loved one.
10. Release of Liability. I release Tribute to America’s Fallen Foundation LLC, its founders, officers, directors, employees, and volunteers from claims arising from the display, handling, transport, or publication of Memorial Materials, except for claims arising from gross negligence or willful misconduct.
11. Governing Law. This consent is governed by the laws of the State of Georgia, without regard to conflict-of-law principles.
